Our purpose is to develop a workbook for self-management of mood problems in individuals suffering from chronic diseases (including COPD, CHF, diabetes, and renal dysfunction). This workbook will draw upon the knowledge and expertise of rehabilitation staff in VCHA, as well as expert researchers and clinicians regarding crucial issues to address and optimal approaches to self-care among chronically ill patients. It represents a tool that would be complementary to the training delivered by the BCCFP Office-Based Self-Management Program. The workbook will be based on well-established cognitive-behavioural change methods, integrating behavioural skills that help reduce mood problems in combination with those that foster improved self-care and illness management. http://www.comh.ca/pchc/ |
